RELEASE FORM

(1.) That Mark Walker (hereinafter referred to as Instructor), has been retained for the purpose of teaching ________________________ the sport of in-line skating and/or skiing. The Instructor has accurately represented that he is licensed in this activity and has fifteen years of experience and expertise in said sport.

(2.) That _______________________ is either over the age of 18 (eighteen) or is the parent/legal guardian of the student of this activity. That the instructor had explained to the student that there is risk of severe physical injury involved in said sport. Furthermore, the undersigns agree that by signature of this release the Instructor is held free and harmless from any injury incurred using the taught fundamentals broughtforth by the Instructor and any injury incurred during the course of these instructions.

(3.) That the instructor has also fully informed the undersigned of the available safety equipment to be worn during the course of such instruction so as to avoid and/or minimize the risk of physical injury. Accordingly, the student and/or guardian is acting within the scope of informed consent in the formation of this relationship.
